decision rights (who may initiate, approve, and stop), controls (how risk is mitigated in
practice), and evidence (what proves we did the right thing) that links models and agents to
verifiable business outcomes.
Objectives. 1) Protect people, data, and the organization; 2) Accelerate safe adoption with
proportionate oversight; 3) Make outcomes measurable and auditable; 4) Create clear accountability for change,
incidents, and benefits realization.
Scope. Governance covers the full lifecycle; use-case intake, data and feature pipelines,
model and prompt assets, agentic workflows and tools, deployment and change management, monitoring and rollback,
vendor and third-party components, and decommissioning/retention.
- Decision rights: Executive Council sets strategy and risk appetite; Oversight Board
approves high-risk tiers and exceptions; Model and Product Owners own adoption, KPIs, and rollback; Data Stewards
own lineage and minimization; Security/Privacy own controls and incidents.
- Controls (minimum viable): risk tiering, PII minimization and redaction at ingress, audit-team and
evaluation packs pre-release, staged promotion with sign-offs, prompt and output logging, drift monitoring with
auto-rollback, exception workflow with SLAs.
- Evidence: immutable logs of prompts, sources, outputs, signed gate checklists, red-team
findings, benefits register with baselines/counterfactuals, and quarterly attestations.
- Cadence: weekly health and risk review, monthly release council, quarterly attestations and
audit pack refresh.
- What governance is not: a blocker or one-time policy document. It is a run loop that
